CQII is pleased to bring you a webinar to review care continuums and their usefulness in developing a deeper understanding of your data.  The HIV Care Continuum, first conceptualized by Ed Gardner has gained wide-spread recognition as a useful tool to understand the major steps in viral suppression.  It has been adopted on a state-wide, region-wide and even a facility level tool. 

 

Our learning objectives are:

  • Understand the Continuum of Care

  • Appreciate the importance of data to the QI process

  • Learn how to identify gaps in care

  • Grasp the concept of diving deeper into data

  • Become familiar with the Continuum as an improvement tool
