A report on an Affordable Care Act demonstration program that examined the outcome of providing people with support for health-related social needs (HRSNs) may have a familiar ring to Ryan White HIV/AIDS Program (RWHAP) agencies with positive results as a result of their work to support care engagement for people with HIV.
The Third Evaluation Report of the Accountable Health Communities Model determined that "[f]orty percent of those who received navigation services had at least one HRSN resolved, including 28% who had all their needs resolved. The report shows how the model decreased Medicare and Medicaid spending for people and reduced emergency department visits and inpatient admissions."
The Accountable Health Communities Model looked at the provision of support for HRSNs to address housing instability, food insecurity, problems with transportation, difficulties with utilities, and interpersonal violence. Analysis groups were put into two categories: navigation assistance versus navigation assistance along with continuous quality improvement.
