Common Patient Assistance Program Application Updated

CPAPA, an application form that can be used to apply for any pharmaceutical assistance program (PAP), has been newly updated. CPAPA, the Common Patient Assistance Program Application, was first developed in 2012 to ease the application process as PAPs have variable eligibility criteria. PAP-eligible individuals include low income uninsured/underinsured individuals who are not eligible for Medicaid, Medicare and ADAP.
The CPAPA application (in fillable form format) comes with a companion document with instructions on how to fill out the form. Both documents were funded by HRSA's HIV/AIDS Bureau under a cooperative agreement with NASTAD.
