From Incarceration to Community Resource Portal

A collection of guides and websites presenting best practices and ideas for connecting individuals to health and other services as they exit incarceration facilities has been compiled by the National Academy for State Health Policy. The webpage, NASHP/HARP Reentry Resource Repository, groups materials into varied categories: general, health, Medicaid, and more. 

Resources on housing appear to be limited. One key exception is HUD's Homecoming: Life After Incarceration, with a series of highlight/awareness-raising videos and associated tools, with a section, Housing, features a Making Housing Happen toolkit.
