The collection of practice transformation resources outlined in the Fall 2019 AETC quarterly newsletter has particular relevance to HRSA Ending the HIV Epidemic Initiative activities currently underway, which count practice transformation and workforce development among the efforts envisioned to reduce new HIV cases, get more people with HIV into care, and end HIV in America.
PT, if you can bear another acronym, seeks to change clinical practices and build the capacity of agencies to take on HIV service work. Earlier work to create Medical Homes is an example of varied PT activities. Among HRSA's PT initiatives is expanding HRSA Health Center work in HIV, which has been underway for a number of years, including under two specific initiatives:
- The Partnerships for Care (P4C) project (2014-2017) produced resources like Integrating HIV Care, Treatment & Prevention Services into Primary Care – A Toolkit for Health Centers, a compilation of resources from the P4C project.
- The Southeast Practice Transformation Expansion Project (2017-2018), which involves regional AETCs in delivery of technical assistance and training, has produced many presentations and courses on testing, linkage to care, and other topics. There's a lot of more information out there. Take a look at the AETC NCRC's broad collection of practice transformation materials, like getting started, assessing an agency, and AHRQ Tools and Resources for Practice Transformation and Quality Improvement.
