Part 2: Empowering Access: Implementing the “No Wrong Door” Approach Using a Community Health Worker Model

The “No Wrong Door” approach connects clients to community-based resources at the first point of contact. The speakers will discuss how HCSEF’s Community Health Advocate model and the adaptation of the “No Wrong Door” approach establishes a foundation for the RWHAP Patient Care program and contributes to patient success along the care continuum.

  • Learn tips and techniques for incorporating a CHA/ CHW approach into RWHAP Patient Care work plans and goals.
