This session will describe how the integration of a patient navigator program and a Management Information System led to increased rates across the HIV continuum of care in a single-provider clinic in El Paso, Texas.

Learning Objectives

  1. Assess the importance of integrating and systematizing data analysis to clinical workflows to bring visibility to potential gaps in care.
  2. Describe how the patient navigator program as well as the Management Information System supports the national HIV/AIDS Strategy primary goals of increasing access to care and optimizing health outcomes of people with HIV and reduce HIV-related health disparities and health inequities.
  3. Identify multi-tiered, data-driven strategies that can be used to address linkage and retention in care in border communities.
