A Whole Team Clinic Approach to Better Planning, Better Screening, Better Outcomes

The use of a pre-visit planner improved performance measures for TB and hepatitis screening. From year ending December 31, 2020, to the year ending December 31, 2023, the team’s number of eligible patients screened for TB increased by 15 percent. In the same period, the number of eligible patients screened for hepatitis C increased by 10 percent.
