The Safe and Sound Return Partnership was implemented from May 2018–March 2020 as part of a RWHAP SPNS initiative. The Safe and Sound Return Partnership was designed to support people recently released from prison or jail. Peer community reengagement specialists worked in tandem with case managers to enable referrals to employment and housing programs for participants before they reentered the community or shortly after their return to the community. Evaluation results showed the program had a positive effect on employment, housing, and linkage and retention in HIV care.

The SPOT offers HIV care, sexual health care, mental health services, and psychiatry services in one location to youth and young adults ages 13–24. Since opening its doors in 2008, The SPOT has served over 20,000 youth and young adults in the St. Louis area. From 2021–2022, viral suppression and retention in care rates among clients increased from 82% to 88% and from 86% to 89%, respectively.

From 2018 to 2019, the intervention used tailored feedback to providers within two New York City emergency departments to increase HIV and hepatitis C virus (HCV) testing and case detection. The intervention was built upon existing evidence that HIV and HCV testing in urban emergency departments is cost effective, and offers a critical access point for people who need testing but lack access to routine care. HIV/HCV testing levels increased during the 30-week intervention period compared to the pre-intervention period.

La coordinación de la atención médica durante la reintegración social pone en contacto a personas encarceladas que tienen VIH con un coordinador de atención médica para facilitar el acceso a los cuidados primarios de VIH y otros servicios y apoyo comunitario durante la transición al momento de salir de la prisión e incorporarse a la comunidad. La TCC tiene como objetivo establecer vínculos vitales entre los cuidados de VIH ofrecidos en la prisión y aquellos ofrecidos a nivel comunitario.

La intervención de Navegación Mejorada de Servicios para Pacientes Mujeres de Color con VIH utiliza navegadores de servicios para pacientes, quienes son personal no médico en clínicas, para reducir los obstáculos en la atención médica y optimizar los cuidados. La intervención fue eficaz en cuanto a mejorar el enlace con los cuidados y la retención, así como también en lo relativo a lograr la supresión viral.

Southern Nevada Rapid stART improves access to immediate prescription of ART in the region. Led by the Office of HIV within Clark County Social Service, Rapid stART was initially developed and implemented at 28 sites to link clients to a prescribing HIV provider within seven days of their HIV diagnosis, and included a Rapid stART Response Team to support clients through the process. The Rapid stART program has increased linkage to HIV medical care, initiation of ART within seven days, and viral suppression.

In 2021, the City of New Orleans employed five CHWs to work with RWHAP Part A-funded agencies on reengaging eligible clients back into care. From February 2021 through April 2022, CHWs attempted to contact 1,215 clients disconnected from care; 207 of these attended a primary care appointment and re-entered care, a 17% success rate at reengaging clients back into care.

The Rapid ART Start Protocol focuses on helping veterans with a new HIV diagnosis access ART and comprehensive treatment as soon as possible. A retrospective cohort study showed positive outcomes, including decreases in the time to first appointment, the time to ART initiation, and the time to reach viral suppression. In addition, patients receiving the intervention were more likely to reach viral suppression compared to patients seen pre-intervention at the same clinic.

Through the Test & Treat Rapid Access (TTRA) Program, clients with a new HIV diagnosis in Miami-Dade County can access ART, receive other services and counseling, start enrolling in RWHAP, and connect to HIV primary care during the initial visit. At Borinquen Health Care Center, one of the clinical sites participating in TTRA, 76% of clients were virally suppressed within three months of receiving a rapid ART start, and 95% were retained in care for 12 months.

Through the Practice Transformation Project, the Native American Community Clinic and Midwest AIDS Education and Training Center developed strategies to increase testing and linkage to care within the American Indian/Alaska Native population, and for those who inject drugs and are experiencing homelessness. These ongoing efforts have increased HIV testing rates by 10 percentage points through harm reduction, community outreach, and culturally sensitive strategies.
