The HOMES Program was implemented by Positive Impact Health Centers (PIHC) from 2018–2020, as part of a RWHAP Part F SPNS initiative. HOMES introduced employment services and bolstered its housing supports for clients, by bringing on two new case managers and strengthening external partnerships. These expanded services enhanced PIHC’s existing 'one-stop shop' model of care leading to improved client outcomes. HOMES participants were more likely to be engaged in care and virally suppressed in 2020 than at the time of program enrollment.

The Safe and Sound Return Partnership was implemented from May 2018–March 2020 as part of a RWHAP SPNS initiative. The Safe and Sound Return Partnership was designed to support people recently released from prison or jail. Peer community reengagement specialists worked in tandem with case managers to enable referrals to employment and housing programs for participants before they reentered the community or shortly after their return to the community. Evaluation results showed the program had a positive effect on employment, housing, and linkage and retention in HIV care.

The SPOT offers HIV care, sexual health care, mental health services, and psychiatry services in one location to youth and young adults ages 13–24. Since opening its doors in 2008, The SPOT has served over 20,000 youth and young adults in the St. Louis area. From 2021–2022, viral suppression and retention in care rates among clients increased from 82% to 88% and from 86% to 89%, respectively.

Northern Nevada HOPES integrated behavioral health into their HIV medical care model. If a patient reports a mental health or substance use concern or screens positive through a mental health or substance use assessment, a co-located behavioral health provider joins the medical provider and patient in the exam room for a triage assessment and next steps. In Nevada, a state with significant shortages of behavioral health providers and long wait times for care, patients with HIV at Northern Nevada HOPES have immediate access to behavioral health services.

The HIV Telehealth Collaborative Care program (HIV TCC) was developed by the Veterans Health Administration Office of Rural Health to provide HIV care via telehealth to veterans living in rural areas. An evaluation of the intervention showed that the availability of telehealth led to increases in rates of viral suppression and retention in care. Based on this success, HIV TCC has been expanded to multiple sites in 16 other states as of 2024.

From 2017 to 2019 Positive Resource Center provided intensive housing and employment counseling to people with HIV who were unstably housed. The Housing Planning Program helped participating clients to make the transition into stable housing and to obtain employment, while remaining engaged in HIV treatment. Housing Planning Program’s efforts resulted in increases in clients obtaining stable housing, and in their incomes from baseline to six months after enrollment.

Family Health Centers of San Diego provides comprehensive health services including HIV care at over 24 clinic locations. This intervention integrates healthcare, housing, and employment services to enhance health outcomes related to HIV. Clients participating in the intervention had improvements in retention in care, viral suppression, housing status, and employment status.

From 2018 to 2019, the intervention used tailored feedback to providers within two New York City emergency departments to increase HIV and hepatitis C virus (HCV) testing and case detection. The intervention was built upon existing evidence that HIV and HCV testing in urban emergency departments is cost effective, and offers a critical access point for people who need testing but lack access to routine care. HIV/HCV testing levels increased during the 30-week intervention period compared to the pre-intervention period.
