This multi-day training will discuss the Transitional Care Coordination (TCC) intervention, which engages people with HIV in the jail setting to support linkage to community-based HIV primary care after incarceration and address client-specific barriers to care, such as housing, transportation, substance use, and mental health.
- Define the core components of the Transitional Care Coordination intervention
- Discuss strategies for successfully building relationships with their local jail systems and the importance of working with people with HIV while incarcerated as well as upon release
- Plan for how training participants will implement the Transitional Care Coordination or components of the intervention in their organization/jurisdiction.
