Review of a unique list of twelve attributes for sustaining gains in quality improvement activities. The authors' work was derived from a successful collaborative in New York City.  While the authors do not assert that every one of the 12 attributes must be followed, they offer guidelines and a framework to help you maintain your quality improvement gains.
