Replication of the Transitional Care Coordination Intervention to Improve Outcomes across the HIV Care Continuum

The rate of HIV among people in US prisons is higher than the general population. This workshop will review an evidence-informed intervention to engage people with HIV in the jail setting. Challenges, strategies, and lessons learned will be discussed to support the adaptation and replication of the evidence-informed intervention.

Learning Objectives

1. Define core components of the Transitional Care Coordination (TCC) intervention.
2. Identify successes, challenges, and lessons learned resulting from the implementation of the TCC intervention at three sites.
3. Discuss tools to support and strategies through which the TCC intervention can be adapted and replicated in jurisdictions across the country.
