Enlaces Por La Salud
Enlaces Por La Salud is an HIV linkage, navigation, and education program for Mexican men and transgender women. The intervention is grounded in a transnational framework for providing cultural context to support the delivery of one-on-one educational sessions to Latina(o/x) people with a new HIV diagnosis, as well as people with HIV who are not yet retained in care. After 12 months, the majority of people participating in Enlaces Por La Salud were retained in care and reached viral suppression.Resource from the RWHAP Best Practices Compilation updated on 02/06/2024

The Alexis Project
The Alexis Project used social network recruiting and engagement, peer navigation, and contingency management to reach and engage transgender women of color with HIV who were not engaged in HIV care. Primero Yo: A Participant-Driven Group Empowering Latina Women with HIV to Find Their Voice
A culturally sensitive, participant-driven, Spanish-language group for Latina women with HIV promoted engagement in HIV services. Many participants had never previously engaged in support services. The increased participation resulted in multiple positive outcomes, including HIV viral load suppression, reduced stigma and isolation, engagement in mental health services, and increased community advocacy.

Re-Engagement into HIV Care: An Urban Communities Model
This session will discuss how to reduce the percentage of HIV patients who are out of care. Participants will learn to use a multi-disciplinary team approach to identify patients who are truly out of care, pinpoint the most effective approach toward contacting patients to be brought back into HIV care, and maintain the patient’s HIV care.
