Case Study - My Health Profile Continuity of Care Record Intervention
HRSA HIV/AIDS Bureau (HAB)
The “My Health Profile” Continuity of Care Record Intervention case study highlights an evidence-informed innovative model of care. This case study focuses on the Retention in Care continuum of care step.
The HIV care continuum refers to the fluid nature of HIV health care delivery and client experiences. This framework has received attention as research has demonstrated the importance of moving clients along the HIV care continuum with the goals of being fully linked, engaged, retained, and virally suppressed. Therefore, finding programs that help clients move along the stages of the continuum—like the one featured here—are particularly important.
This case study is part of The Improving Health Outcomes: Moving Patients Along the HIV Care Continuum and Beyond series that includes a fuller manual, additional case studies, pocket guides, and TA webinars.
This case study highlights:
- Intervention steps at-a-glance to provide an overview of featured activities,
- Resource checklists outlining necessary capacity for replication,
- Staffing table with personnel overviews, and
- Narrative sections to walk readers through processes for replication.
The case study is aimed at health care providers, social support providers, and technical assistance trainers across all health settings. This intervention was initially implemented in a Ryan White HIV/AIDS Program-funded site, so those grantees who may be seeing similar populations, and who are facing similar challenges, may also benefit greatly.
