Micro Elimination of Hepatitis C Among People with HIV, New York City, 2016-2019

The New York City Health Department data-to-care model to eliminate Hepatitis C in people with HIV uses surveillance data to target health care practices for training and clinical practice facilitation, to identify patients for outreach and linkage to care, and to monitor progress toward elimination.
