Outcomes of a Transitional Care Coordination Intervention Linking People with HIV in Jails to Community HIV Care and Services

Description

Three sites were funded in 2016 as part of Special Projects of National Significance Dissemination of Evidence-Informed Interventions initiative to implement an evidence-informed intervention designed to link people with HIV in jail to community HIV care using a transitional care coordinator. Presenters will discuss the barriers and facilitators related to linking to care and achieving and maintaining viral suppression post-release. Presenters will also discuss strategies for successful collaboration with local jails and best practices for providing high-quality HIV patient navigation and case management as an external contractor within a jail setting. Presenters will describe how the intervention was adapted to meet the specific needs of the jail and the community system of care. Resources, tools, and the Care and Treatment Intervention manual for the intervention will be reviewed and shared with participants.

  1. Describe the implementation evaluation outcomes – barriers and facilitators at before and during implementation.
  2. Discuss the client health outcomes and factors associated with linkage and viral suppression.
  3. Share resources and tools to adapt and replicate the intervention.
