Positive Health Clinic successfully completed a quality improvement project related to our EMR, EPIC. The focus of this project was streamlining data collection, data utilization, and reporting processes in the EMR. The enhancements to our EMR yielded improvements in three areas: communication across disciplines, formalized treatment plans, and data reporting.

The COVID-19 public health emergency (PHE) exacerbated homelessness and housing instability. NY EMA Planning staff worked collaboratively to address homelessness as a driving force of poorer health outcomes for people living with HIV. This presentation will illustrate community planning processes for addressing housing-related barriers among RWHAP consumers during the PHE.
The percentage of people with HIV who could become pregnant and lacked designated form of contraception rose from 13 percent to 22.5 percent between April 2020-April 2021 in our Southeastern RWHAP clinic. We developed a reproductive health visit to reduce the rate of those with no contraception plan from 22.5 percent to 17 percent by July 2022.
A panel of parents who have domestically and internationally adopted youth with HIV discuss the complexities that arise from disclosure, care coordination/engagement, sexuality and dating, and transition to adult care while sharing their individual parenting strategies and struggles.
California’s AIDS Drug Assistance Program (ADAP) and Pre-Exposure Prophylaxis Assistance Program (PrEP-AP) provide access to life-saving medications and insurance assistance for eligible California residents living with or at risk of acquiring HIV. This presentation will explore the efficacy of various program policy changes implemented by California during the COVID-19 pandemic.
Minimize to Maximize. The Utah RWHAP Part B Program follows Franklin Covey’s 4 Disciplines of Execution to collaborate with a medical case management provider to minimize the number of goals to maximize success. A virtual scoreboard monitors measures and commitments within a structured cadence of self-accountability to improve outcomes.
Review of rapid ART features to reduce time to ART from 7 to 1.6 days, including pre-visit benefits navigation, manufacturer discount cards, prescribing to in-house pharmacies, email distribution lists, and post-prescriptive follow-up.

Columbia University College of Dental Medicine has partnered with community partners to implement a dental care delivery system for PLWHA in NYC. A week-long Service Learning Rotation was integrated into the third-year dental student curriculum; data analysis from this pilot year will be used for program quality improvement and modification.
Significant challenges exist in vaccinating people with HIV for COVID-19. Our RWPC program set out to implement and evaluate a vaccine initiative for COVID-19 for people with HIV. We achieved vaccine rates of 78 percent being fully vaccinated by December 1, 2021 and 30 percent having a third dose.
