Boston Healthcare for the Homeless Program uses an innovative care model, designed from the margins, to meet the complex needs of people living at the intersections of HIV, homelessness, substance use disorder, and incarceration. Such models hold promise for closing HIV care and prevention equity gaps for this hyper-vulnerable group.

This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. Pharmacists' involvement in pre-exposure prophylaxis will also be reviewed.

Leadership Institute Session 1 focuses on the leadership of people with HIV and will cover the role and history of meaningful involvement of people with HIV. It will explore culturally responsive leadership, transformational leadership, and cultural humility. Facilitators will demonstrate formal and informal leadership skills. People with HIV are encouraged to attend.

Participants will explore different forms and manifestations of stigma, the impact of stigma on health outcomes, and the tenets of intersectionality. There will be discussion on improving factors contributing to stigma and intersectionality that impact the HIV service delivery system. People with HIV are encouraged to attend.

This session will use stories to describe a small 'village,' a model to assist in, prevent relapse of, and remove barriers to care for patients with opioid use disorders. Motivational interviewing techniques are applied in the program evaluation context, using examples from Ryan White programs. Using motivational interviewing, the evaluator leads the dance in the desired direction while incorporating stakeholder input.
