This session will explain how Texas collects gender identity data for people with HIV and will examine disparities in care and health outcomes for transgender Texans living with HIV with an additional focus on the Latinx and black communities in order to understand how to better meet the needs of transgender people with HIV.

This presentation will look at the introduction of a clinical pharmacist in an urban, Ryan White-funded clinic, the impact on medication adherence, and the viral suppression and demographic trends observed over four years.

Participants will explore different forms and manifestations of stigma, the impact of stigma on health outcomes, and the tenets of intersectionality. This workshop will present the 6Ms, a simplified approach to geriatric principles for HIV care: Mobility, Mentation, Medications, Matters Most, Multicomplexity, and Modifiable. The role of the 6Ms in clinical care and identification of older adults at greatest need will be considered; clinical models of geriatric care with potential for widespread implementation will be presented.

Presenters will describe a medical community collaboration to resolve care gaps and improve patient health outcomes through the co-location of health home care management by a community-based organization (CBO) within medical clinics. New HIV infections continue to rise among members of the Latinx community, especially youth. To reverse this increasing trend, it is important to recognize the social, economic, linguistic, and cultural barriers that hinder the prevention and treatment efforts for HIV when developing an outreach strategy.

Prior analyses among transgender women receiving Ryan White Part A services in New York City have found evidence of disparities in HIV outcomes. This presentation will focus on viral suppression and the New York Eligible Metropolitan Area’s efforts to address care continuum outcome disparities affecting clients of transgender experience.

People with HIV 50 years and older is a rapidly increasing population with unique needs. The presentation summarizes a population-based approach to assess those needs and to respond with training and other resources. Presenters will describe the implementation of the approach by the New York City EMA in partnership with the Northeast/Caribbean AETC.
