Leveraging a Data to Care Approach to Cure HCV among People with HIV: Implementation Manual and Data-to-Care Tools

Activities of jurisdictions and their partner clinics to implement Hepatitis C Virus (HCV) Data to Care project activities, based on their previous experience, data management infrastructure, ability to share data, and staffing resources.

The implementation manual summarizes approaches, lessons learned, and best practices for supporting the replication of efforts, and lists key resources for health department jurisdictions to start implementation.

HCV D2C Co-Infection Clearance Cascade

The HCV co-infection clearance cascade is a tool to help jurisdictions to visualize diagnosis and treatment milestones to identify gaps in care for individuals with HCV/HIV co-infection. By monitoring progress from initial infection to cure, jurisdictions can evaluate efforts that increase testing and linkage to care.

HCV D2C Case Conferencing Tool

Jurisdictions partnered with RWHAP clinics to address gaps in HCV clearance cascades by utilizing an Excel-based Case Conferencing Data Tool, enabling identification of clients needing HCV care and tracking clinical outcomes to measure progress. This approach involves a case conferencing process offering insights and guidance for implementing similar processes in other jurisdictions.

SAS Code for Populating the Clearance Cascade Template

This SAS code may help jurisdictions populate the HCV clearance cascade template. There is no standardized national HCV reporting system, so each jurisdiction has to develop code that aligns with internal data management and naming conventions. However, this sample SAS code can highlight potential approaches.
