Description

Presenters will describe a New York City collaborative pilot project utilizing Regional Health Information Organizations (RHIOs) to locate lost-to-care people with HIV. Lost-to-care encounter alerts provide community-based organizations with actionable, real-time data to supplement their ‘classic' care engagement efforts. The model offers a potentially scalable, cost-effective strategy for patient re-engagement efforts on a population level.

Learning Objectives

  1. Gain insight into how RHIOs can facilitate individual- and population-level care engagement.
  2. Learn how a unique multi-stakeholder project among medical facilities and community partners can effectively address the care engagement needs of vulnerable populations.
  3. Learn best practices for outreach and linkage to care efforts and the benefits of combining social service and clinical data in RHIOs in response to alerts.
