Description

This presentation offers insights from a successful quality improvement (QI) project to improve three-day linkage to care in a large urban federally qualified health center (FQHC) network. Attendees will learn concrete strategies to improve linkage to care, including changes to policy and workflow, as well as new approaches to using data for QI.

Learning Objectives

  1. List three changes to workflows and procedures that can be made to support timely linkage to care.
  2. Describe two strategies for visualizing linkage-to-care data and sharing linkage-to-care data with providers, staff, and stakeholders.
  3. Participants will be able to discuss three to five factors that can support the success of a QI project.
