This workshop will introduce a practical approach for focused and inclusive conversations about evaluation and HIV program outcomes. Motivational interviewing techniques are applied in the program evaluation context, using examples from Ryan White programs. Using motivational interviewing, the evaluator leads the dance in the desired direction while incorporating stakeholder input.

This workshop will provide a game demonstration. The game was developed to address the common scenarios that occur which cause stress and confusion in the battle to weigh client needs against agency ethos and program requirements. The game promotes motivational interviewing skills for problem-solving with consumers and coworkers alike.

No-show rate is a strong and independent predictor of patients falling out of care at the partnership that cares for 1,700 people with HIV in Philadelphia. A pilot study was created to address every no-show event in an effort to prevent patients from falling out of care.

The PATHways Program at Vanderbilt Comprehensive Care Clinic is an interdisciplinary, nursing-led program designed to reach people who are traditionally challenging to engage in care. We have helped over 80% of participants achieve viral suppression. We offer our approach as a case study of a successful innovative model of care.

Presenters will describe a medical community collaboration to resolve care gaps and improve patient health outcomes through the co-location of health home care management by a community-based organization (CBO) within medical clinics. Panel members will share successes and challenges of implementation of the project and integrating a CBO care team into a medical clinic.
