The Northeast/Caribbean AIDS Education and Training Center HIV Health Information Exchange (HIE) Initiative aims to empower HIV care networks to utilize the potential of emerging data exchange models toward improving HIV outcomes and ending the epidemic. A community health center uses a mobile health unit to provide integrated substance use disorder (SUD) and HIV care and treatment for those suffering from housing instability. This low-barrier, mobile, integrated care model, supported by intensive case management and outreach, has made a significant impact on engagement, antiretroviral treatment (ART) initiation, and viral suppression.

Three sites in the Special Projects of National Significance Black Men Who Have Sex with Men (BMSM) Initiative have developed and implemented innovative technology innovations to engage BMSM in care. These include apps, telemedicine, social media, and texting services that meet clients where they are and provide access to services. This presentation will describe product selection and early implementation experiences.
