The New York City HIV Care Coordination Program is a structural intervention that combines multiple strategies, including multidisciplinary care coordination, patient navigation, and personalized health education to address client medical and social needs. Enlaces Por La Salud
Enlaces Por La Salud is an HIV linkage, navigation, and education program for Mexican men and transgender women. The intervention is grounded in a transnational framework for providing cultural context to support the delivery of one-on-one educational sessions to Latina(o/x) people with a new HIV diagnosis, as well as people with HIV who are not yet retained in care. The Village Project
The Village Project is an intensive case management-based intervention that harnesses peer navigation and integrated behavioral health services to improve the health outcomes of young Black gay, bisexual, and men who have sex with men. Huntridge Rapid Start Initiative
The Huntridge Family Clinic launched the Rapid Start Initiative to provide same-day ART treatment and comprehensive case management to clients with a new diagnosis of HIV. Bottom-Up Project
The Bottom-Up Project is a multi-organizational initiative focused on leveraging health information exchange data and peer navigation. Using real-time clinical data, in combination with linkage to HIV care and social services, the Bottom-Up Project locates and reengages people with HIV who are not currently in medical care and are not virally suppressed. Using a Multidisciplinary Approach to Improve Retention in HIV Care
MacGregor Infectious Diseases, a hospital-based clinic affiliated with the Hospital of the University of Pennsylvania, implemented a multidisciplinary approach to strengthen outreach to clients and improve care retention. Creating a Culture of Trauma-Informed Care
Lehigh Valley Health Network Comprehensive Health Services implemented a trauma-informed approach to care delivery, including training staff on how trauma can affect people’s health and how microaggressions in healthcare environments can potentially trigger trauma responses. Co-Located HIV and SUD Care on a Mobile Unit for Homeless People with HIV and SUD
A community health center uses a mobile health unit to provide integrated substance use disorder (SUD) and HIV care and treatment for those suffering from housing instability. This low-barrier, mobile, integrated care model, supported by intensive case management and outreach, has made a significant impact on engagement, antiretroviral treatment (ART) initiation, and viral suppression.

Using Innovative Technology Strategies to Engage BMSM with HIV in Care
Three sites in the Special Projects of National Significance Black Men Who Have Sex with Men (BMSM) Initiative have developed and implemented innovative technology innovations to engage BMSM in care. These include apps, telemedicine, social media, and texting services that meet clients where they are and provide access to services. This presentation will describe product selection and early implementation experiences.
