This presentation will describe two innovative mental health quality improvement initiatives for youth. The presentation will focus on the specifics of delivering brief mental health in-clinic interventions and the development and implementation of youth peer support groups within the pediatric HIV program in the ‘hot spot' of the national HIV epidemic, Washington, D.C.

Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

This session will give information on the RWHAP SPNS Social Media Initiative (SMI), which used social media and digital technology to improve HIV outcomes for young people. University Health System’s peer engagement initiative employs peers, alongside Patient Navigators, to provide support and improve linkage and retention to care for youth, women, infants, and children. Group support activities and a chat-enabled website are two of the methods used to continually engage our population of focus.

This workshop will share youth-tailored innovative approaches, such as integrated mental health and clinical nutrition services for peer support, facilitated transportation conducted by case managers via a rideshare program for youth clients, and a grading system to assess individual readiness and help guide transition to adult care.

As part of the SPNS Social Media Initiative (SMI), three Ryan White recipients describe their experiences with text messaging, both automated and real-time, to increase engagement in care and the number of medical appointments kept, and improve viral suppression rates with an emphasis on engaging young men who have sex with men and trans women of color.

Community Health Worker (CHW) models are a strategy to reduce inequities in HIV. This presentation will describe an evaluation of 397 CHW program participants, with findings on the impact on barriers to care and clinical outcomes, as well as the relationship among unmet needs, CHW encounters, and clinical outcomes.
