Staff will present a population-based, data-driven approach to the Minority AIDS Initiative (MAI) funding designed to improve program engagement and retention to reduce health disparities and maximize health outcomes. As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

Linkage Navigation Program provides an immediate connection to same-day medical care and medical case management services, promoting timely linkage to care and services, and tailored individualized care operationalizing initiatives such as Test N Treat and Test N PrEP with positive health outcomes for Ryan White clients.

Successes and lessons learned will be shared from three metropolitan areas on incorporating STI testing and treatment for prevention clients within an HIV medical home setting, along with how offering STI treatment impacts early identification service outcomes (e.g., HIV positivity rates, linkage to care referrals, and the rapid start of HIV treatment).

A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

Cooper EIP Expanded Care Clinic in Camden, NJ has incorporated comprehensive Addictions Medicine (AM) care as part of its treatment model for people with HIV. A cohort of Substance Use Disorder (SUD) diagnosed patients displayed significant levels of VLS after enrolling in AM services.

Through creative innovation, multiple approaches have been employed to assist patients with medication adherence. This presentation will look at the introduction of a clinical pharmacist in an urban, Ryan White-funded clinic, the impact on medication adherence, and the viral suppression and demographic trends observed over four years.

Oklahoma has been classified by the Department of Health and Human Services as one of the seven states with a high rural HIV burden. Test-and-treat protocols are feasible within high-volume HIV clinics which serve rural and underserved communities to minimize the time to the first appointment to decrease time to viral load suppression.
