Navigator Case Management for People Leaving Jail
The Navigator Case Management intervention helps people with HIV who are incarcerated and are leaving to return to the community. The intervention uses harm reduction, case management, and motivational interviewing techniques to promote healthy behaviors. Postpartum Retention and Engagement Quality Improvement Initiative
The University of Mississippi Medical Center implemented a Postpartum Retention and Engagement Quality Improvement Initiative in 2017 to improve linkage to care, retention in care, and viral suppression among postpartum women with HIV. This intervention uses a combination of care coordination, printed materials, case management services, and improved collaboration and coordination between the Adult Special Care Clinic, which provides comprehensive HIV medical care, and a Perinatal HIV Program. Oral Health Data in CAREWare: Process optimization – the Baltimore EMA Experience
Oral health of RWHAP participants remains a critical area of unassessed unmet need. Normative data needs and SWOT analyses indicated need for process improvement strategies involving providers for better data quality. Voluntary oral health data reporting is a key barrier to understanding/tracking the oral health status of RWHAP participants.

Addressing Opiate Use through Practice Transformation: Implementing Dashboard Reports to Improve Panel-Based Care

Housing First as a Key Strategy to End the HIV Epidemic
Housing First is an effective, cost-efficient, evidence-based best practice that demonstrates improved health outcomes and care utilization for people experiencing homelessness, including people with HIV (PWH). Ending the HIV Epidemic in the U.S. (EHE) initiative is a unique opportunity for HRSA-funded Jurisdictions to support the uptake and implementation of the Housing First model in their communities to prevent new HIV diagnoses and engage people with HIV who are outside the current system of care.
