In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Boston Healthcare for the Homeless Program uses an innovative care model, designed from the margins, to meet the complex needs of people living at the intersections of HIV, homelessness, substance use disorder, and incarceration. Such models hold promise for closing HIV care and prevention equity gaps for this hyper-vulnerable group.

This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. Pharmacists' involvement in pre-exposure prophylaxis will also be reviewed.

Boston Healthcare for the Homeless Program will share its innovative approach toward the micro-elimination of Hepatitis C (HCV) for homeless-experienced people with HIV. Team members will share data around cure rates, reinfection, and engaging with marginalized patients, and discuss leveraging existing Ryan White infrastructure and resources for such efforts.

Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations
