Providing HIV Services to People Who Are Incarcerated
The Maricopa Jail Project was implemented by five jails to decrease the wait time between incarceration and/or diagnosis to the start of treatment, and to better support clients to reach viral suppression. Maricopa hired a nurse practitioner to manage access and case manage across the jail system. Primero Yo: A Participant-Driven Group Empowering Latina Women with HIV to Find Their Voice
A culturally sensitive, participant-driven, Spanish-language group for Latina women with HIV promoted engagement in HIV services. Many participants had never previously engaged in support services. The increased participation resulted in multiple positive outcomes, including HIV viral load suppression, reduced stigma and isolation, engagement in mental health services, and increased community advocacy.

The Benefits of Interdisciplinary Teams in Caring for Patients with HIV
With integrative, patient-centered care, the organization’s interdisciplinary team approach to caring for patients with HIV has achieved a retention rate of 95% and the viral load suppression of patients with HIV to 95%.

Outcomes of a Transitional Care Coordination Intervention Linking People with HIV in Jails to Community HIV Care and Services
Three sites were funded in 2016 as part of Special Projects of National Significance Dissemination of Evidence-Informed Interventions initiative to implement an evidence-informed intervention designed to link people with HIV in jail to community HIV care using a transitional care coordinator. Presenters will discuss the barriers and facilitators related to linking to care and achieving and maintaining viral suppression post-release.
