Summary: H.O.M.E.S. (Housing Opportunities, Medical, & Employment Services) is a housing-first model to help people with HIV address their housing instability while working towards obtaining employment and maintaining consistent medical care. Every patient completes an individual service plan, developed in partnership with their case manager.
