Services addressing social determinants of health are provided in corrections facilities and upon release. Three sites in the Special Projects of National Significance Black Men Who Have Sex with Men (BMSM) Initiative have developed and implemented innovative technology innovations to engage BMSM in care. These include apps, telemedicine, social media, and texting services that meet clients where they are and provide access to services. This presentation will describe product selection and early implementation experiences.
