In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Boston Healthcare for the Homeless Program uses an innovative care model, designed from the margins, to meet the complex needs of people living at the intersections of HIV, homelessness, substance use disorder, and incarceration. Such models hold promise for closing HIV care and prevention equity gaps for this hyper-vulnerable group.

The Alabama Quality Management Group (AQMG) is a consortium of quality leaders representing all Alabama RWHAP Part C and D clinics, over 80% of Alabama's HIV-diagnosed population. Data for Care Alabama (D4C-AL), a CDC-funded retention-in-care project, uses risk-stratification based on missed visits. It is currently being implemented at one site but will continue to be implemented over time.

Boston Healthcare for the Homeless Program will share its innovative approach toward the micro-elimination of Hepatitis C (HCV) for homeless-experienced people with HIV. Team members will share data around cure rates, reinfection, and engaging with marginalized patients, and discuss leveraging existing Ryan White infrastructure and resources for such efforts.

Ryan White Part B fiscal monitors in Texas developed processes and tools to identify, track, and report program income generated through the 340B program and other sources of revenue. Tools include a spending plan template and an allocation worksheet, which will be shared with other RWHAP recipients in this workshop.

This presentation features an interactive assessment activity with Specific Measurable Achievable Relevant, and Timely (SMART) goal action planning to equip attendees in identifying opportunities to improve linkage and retention in care practices. Concrete strategies from the field and evaluation tips will be shared alongside critical information on operationalizing integrated care models to best serve individuals experiencing homelessness.

The key to ending the epidemic in the United States is the employment of techniques that pinpoint where HIV infections are spreading rapidly and mobilizing resources for HIV services. The Ursuline Sisters HIV/AIDS Ministry in Youngstown, Ohio, began an emergency shelter and started providing housing services for people with HIV who were homeless. This session will discuss the lessons learned, best practices, and gains made toward helping the most marginalized clients achieve viral load suppression.

Funded by the HRSA Special Projects of National Significance initiative, Project CORE is a coordinated housing, employment services, and HIV care intervention that aims to increase employment and income outcomes of persons with HIV who are currently receiving services through HOPWA and/or RWHAP. The purpose of this presentation is to describe Project CORE, its strategies, and preliminary findings.

The Housing Independence Through Employment (HITE) program assists highly motivated individuals with achieving their highest level of economic and housing independence. For a period of 24 months, participants receive assistance with 100% of their monthly rent and pay 30% of their income to an escrow account while receiving robust support services.
