Bottom-Up Project
The Bottom-Up Project is a multi-organizational initiative focused on leveraging health information exchange data and peer navigation. Using real-time clinical data, in combination with linkage to HIV care and social services, the Bottom-Up Project locates and reengages people with HIV who are not currently in medical care and are not virally suppressed. Through this collaboration, over half of patients on the lost-to-follow-up list were found and invited to enroll in the linkage to care/reengagement program.Resource from the RWHAP Best Practices Compilation updated on 01/03/2024

Using a Multidisciplinary Approach to Improve Retention in HIV Care
MacGregor Infectious Diseases, a hospital-based clinic affiliated with the Hospital of the University of Pennsylvania, implemented a multidisciplinary approach to strengthen outreach to clients and improve care retention. Providing HIV Services to People Who Are Incarcerated
The Maricopa Jail Project was implemented by five jails to decrease the wait time between incarceration and/or diagnosis to the start of treatment, and to better support clients to reach viral suppression. Maricopa hired a nurse practitioner to manage access and case manage across the jail system. Co-Located HIV and SUD Care on a Mobile Unit for Homeless People with HIV and SUD
A community health center uses a mobile health unit to provide integrated substance use disorder (SUD) and HIV care and treatment for those suffering from housing instability. This low-barrier, mobile, integrated care model, supported by intensive case management and outreach, has made a significant impact on engagement, antiretroviral treatment (ART) initiation, and viral suppression.
