Bottom-Up Project
The Bottom-Up Project is a multi-organizational initiative focused on leveraging health information exchange data and peer navigation. Using real-time clinical data, in combination with linkage to HIV care and social services, the Bottom-Up Project locates and reengages people with HIV who are not currently in medical care and are not virally suppressed. The YGetIt? Program engaged youth and young adults with HIV in care through three components: a health management mobile application (GET!), Peer Engagement Educator Professionals (PEEPs), and a graphic serial (Tested). Performance Measures and the HIVQM
This webinar is open to all Ryan White HIV/AIDS Program recipients and subrecipients who are interested in using the HIV Quality Measures (HIVQM) Module. The HIVQM Module is a tool developed by HAB that allows recipients and subrecipients to enter their HAB performance measures and continually monitor their progress in improving health outcomes for people with HIV. HIVQM Module users can generate summary reports and compare their progress with other users regionally, nationally or by RWHAP Part.

INFINI-T
INFINI-T is a program designed to reach young transgender women of color with HIV and retain them in care through peer advocates who provided case management. These peer advocates facilitated social support sessions with the goal of alleviating the negative impacts of psychosocial factors like history of trauma, stigma, and discrimination. Rise
Rise was implemented to address ART adherence issues among Black people with HIV. Through Rise, peer counselors used motivational interviewing techniques, leveraged Medication Event Monitoring System data to monitor and promote adherence, and connected participants to support services to address unmet needs. Practice Transformation Project at the Native American Community Clinic
Through the Practice Transformation Project, the Native American Community Clinic and Midwest AIDS Education and Training Center developed strategies to increase testing and linkage to care within the American Indian/Alaska Native population, and for those who inject drugs and are experiencing homelessness.
