The STAR Program, a Ryan White Part C program, initiated a 30-minute, weekly televised TV health magazine that engages the community across multiple social media and television platforms with a focus on health particularly HIV treatment and Prevention, reaching young adults. A case study of one Part B subrecipient improved linkage and retention rates through the innovative use of medical transportation, housing services, and food bank and home-delivered meals. The presentation will share lessons learned and propose strategies to replicate these services elsewhere.

Managing the needs of people with HIV can be difficult and intensive. The importance of caring for oneself is often overlooked by program staff and can threaten their well-being. Staff who provide non-medical case management and assistance in finding employment and housing were interviewed to determine key areas of concern.

Prism Health North Texas will share the challenges and successes of integrating non-medical case management workflow processes into an integrated electronic health record system. The presenter will discuss workflow processes, assessments that allow non-medical case managers to gather required information and determine outcomes and identify patients appropriate for program graduation.

This workshop will describe how changes to an electronic health record (EHR) can capture programmatic outcomes data to evaluate program effectiveness, the benefits of developing custom tools within EHRs, how these EHR tools were developed, and how they have been integrated into program and evaluation work processes.

The Los Angeles County Department of Health compared change in acuity level from initial assessment to reassessment among Los Angeles County Medical Care Coordination (MCC) clients. At reassessment, 2,361 clients (50%) had a significant reduction in acuity. MCC is an effective strategy to reduce medical and psychosocial acuity in addition to improving HIV care continuum outcomes.

The key to ending the epidemic in the United States is the employment of techniques that pinpoint where HIV infections are spreading rapidly and mobilizing resources for HIV care and treatment. Engaging the community and including faith institutions are key to improve access to care, treatment, and adherence. This session will highlight how three projects are working with implementing partners in the rural South.

As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

The session will discuss lessons learned from the Rhode Island's path to the AIDS Drugs Assistance Program (ADAP) automation through the implementation of the Lifia Portal. The Lifia portal is a tool designed to manage client enrollment, eligibility and benefits, streamline and take an ADAP program to the next level.

This sub-study compared viral suppression between SPNS participants and non-participants at the Meharry site. The SPNS study enrolled women of color newly who were recently diagnosed with HIV or lost to care. There was no statistically significant difference in viral suppression after controlling for demographic and clinical factors.
