Managing the needs of people with HIV can be difficult and intensive. The importance of caring for oneself is often overlooked by program staff and can threaten their well-being. Staff who provide non-medical case management and assistance in finding employment and housing were interviewed to determine key areas of concern.

Expanding Jail Services & Improving Health for Incarcerated People Using HRSA Policy 18-02
Maricopa County’s Part A Program used HRSA Policy 18-02, which defines allowable services for incarcerated individuals, to expand services in county jails. This session provides a clinical and administrative perspective on the processes, forms, and services that decreased time from jail-based diagnosis to treatment and viral load suppression.

Expanding Medication Access: Iowa's Jail Assistance Program
In December 2019 the Iowa ADAP launched its Jail Assistance Program to extend HIV medication access to county jails. The ADAP partnered with a local jail to create a program that could be replicated across all of Iowa’s 99 counties. This presentation will focus on program development, utilization, and lessons learned.

Change in Acuity Level Between Assessments among Los Angeles County Medical Care Coordination Clients
The Los Angeles County Department of Health compared change in acuity level from initial assessment to reassessment among Los Angeles County Medical Care Coordination (MCC) clients. At reassessment, 2,361 clients (50%) had a significant reduction in acuity. MCC is an effective strategy to reduce medical and psychosocial acuity in addition to improving HIV care continuum outcomes.

Criminal Justice Initiative: Addressing Continuum of Care for People with HIV and/or hepatitisC from Incarceration to Re-Entry
Panelists will describe collaborative prevention and support services provided to people with HIV and/or hepatitis from incarceration to re-entry. Services addressing social determinants of health are provided in corrections facilities and upon release. Panelists include the program director, staff from a correctional medical facility, and community-based organizations working inside and outside of correction facilities.
