As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

The session will discuss lessons learned from the Rhode Island's path to the AIDS Drugs Assistance Program (ADAP) automation through the implementation of the Lifia Portal. The Lifia portal is a tool designed to manage client enrollment, eligibility and benefits, streamline and take an ADAP program to the next level.

In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Workshop attendees will be introduced to the end+disparities ECHO Collaborative, the largest virtual community of practice of its kind. Using data to identify disability-related needs of RWHAP provider agencies and clients to assess outcomes along the HIV Care Continuum, the New York HIV Planning Council developed with recipient support recommendations to better accommodate people with HIV with disabilities living in the New York EMA to improve service delivery, engagement, and viral suppression.

The workshop will provide perspective on effectively engaging consumers in ending the HIV/AIDS epidemic. The HIV surveillance-based Agency-level Viral Suppression Report (AVSR) is intended to help RWHAP agencies understand viral suppression trends among their client population. Redesigning the AVSR to include viral suppression comparisons between additional client subgroups and vulnerable populations can expand agencies' capacity to identify health disparities and achieve equitable health outcomes.

Community Health Worker (CHW) models are a strategy to reduce inequities in HIV. This presentation will describe an evaluation of 397 CHW program participants, with findings on the impact on barriers to care and clinical outcomes, as well as the relationship among unmet needs, CHW encounters, and clinical outcomes.
