Positive Love: Improving Health Outcomes of Young, Black MSM with HIV in the Deep South
The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

National Implementation of a Text-Messaging Intervention to Improve Antiretroviral Adherence among Youth with HIV
This workshop will explore two sites' (Brooklyn and Detroit) experience with the implementation of an evidence-informed intervention for black men who have sex with men (MSM) with HIV. Using Shared Measures among the AQMG to Increase Retention-In-Care among Alabama's Ryan White Clinics
The Alabama Quality Management Group (AQMG) is a consortium of quality leaders representing all Alabama RWHAP Part C and D clinics, over 80% of Alabama's HIV-diagnosed population. Data for Care Alabama (D4C-AL), a CDC-funded retention-in-care project, uses risk-stratification based on missed visits. It is currently being implemented at one site but will continue to be implemented over time.

Utilizing an HIV Clinical Pharmacist to Improve Patient Outcomes
This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. Responding to the Global HIV Epidemic in a Post-Industrial City: The Brockton Model
This session will examine the components of a successful HIV treatment and care model targeting foreign-born and other vulnerable patient populations. Community Health Workers Improve Viral Load Suppression in Non-Treatment Adherent Populations to End the Epidemic
Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations
