People with HIV 50 years and older is a rapidly increasing population with unique needs. The presentation summarizes a population-based approach to assess those needs and to respond with training and other resources. Presenters will describe the implementation of the approach by the New York City EMA in partnership with the Northeast/Caribbean AETC.

This workshop will present the 6Ms, a simplified approach to geriatric principles for HIV care: Mobility, Mentation, Medications, Matters Most, Multicomplexity, and Modifiable. The role of the 6Ms in clinical care and identification of older adults at greatest need will be considered; clinical models of geriatric care with potential for widespread implementation will be presented.

This workshop will help participants assess and improve their collaborative process with Integrated HIV Prevention and Care Plan development and implementation. Presenters will describe challenges and facilitators with cross-Part collaboration on integrated planning, including the development, implementation, monitoring and improvement of integrated plans, and alignment with other planning efforts.

Ten fact sheets on client populations getting care from the Ryan White HIV/AIDS Program (RWHAP) a
HIV planning groups are responsible for overseeing the planning and implementation of HIV prevention and care activities within communities. Historically, prevention and care programs have operated separate planning groups. RWHAP Part A and Part B recipients and CDC-funded prevention jurisdictions have conducted parallel planning activities, oftentimes with little collaboration or coordination. As a way to reduce duplicative planning activities and streamline the work of planning groups, a number of jurisdictions have integrated their HIV planning groups.

In this webinar, panelists discussed the ways in which power imbalances can manifest in HIV planning bodies; how their planning bodies have sought to promote equity; and strategies that can be implemented to foster equity and mitigate power imbalances based on race, education, age, and socioeconomic status.

During this workshop, IHAP TAC presented strategies and best practices to support successful implementation, monitoring, and evaluation of Integrated Plans. The session focused on the value of maintaining active engagement and involvement from planning bodies, community members, and other stakeholders that participated in the plan development process.
