Innovative, Integrated Care: Two Unique Deliveries of Telepsychiatry Services
Telepsychiatry has proven to provide better access and higher-quality care to patients who need psychiatric care as well as for those who have varying circumstances that make it difficult to engage in this service. Vivent Health has successfully integrated telepsychiatry within its medical home model. With two different service delivery methods, this presentation will look at the benefits of telepsychiatry for people with HIV, as well as the unique delivery methods Vivent Health provides in Wisconsin and Colorado.

Promoting Improved HIV Health Outcomes in Clients Experiencing Unstable Housing
This session will present strategies to improve viral suppression in clients experiencing unstable housing and homelessness. This includes both a recipient serving homeless clients and a RWHAP AIDS Education and Training Center that trains clinicians on strategies to serve clients experiencing homelessness. HCV Linkage, Screening, and Treatment among Co-infected Clients: Enhancing CAREWare and Electronic Health Records
As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

Leveraging HRSA and HUD Funding to Improve Outcomes for People Who Are Unstably Housed

Reducing Barriers to Care for People with HIV with Visible and Invisible Disabilities
Using data to identify disability-related needs of RWHAP provider agencies and clients to assess outcomes along the HIV Care Continuum, the New York HIV Planning Council developed with recipient support recommendations to better accommodate people with HIV with disabilities living in the New York EMA to improve service delivery, engagement, and viral suppression.
