Through the Southern Initiative, CAI supported four Part A agencies in the South in integrating a Community Health Worker (CHW) model to address disparities in HIV outcomes. The session will describe the development of systems to collect, report, and use real-time data to track implementation progress and client outcomes. Results reveal promising client outcomes.

University of Mississippi Medical Center utilized known barriers to care for postpartum women with HIV and leveraged that information to schedule individualized interactions with pregnant and postpartum women with HIV. This low-cost, low-effort initiative resulted in statistically significant improvements in both retention in care and viral suppression rates in postpartum women with HIV.

Become familiar with tools and resources to help identify and reduce viral suppression disparities among key populations living with HIV. This session showcases the activities undertaken by the Ryan White Wellness Center in Charleston, South Carolina, that helped eliminate viral suppression disparities among its young men of color who have sex with men.

This sub-study compared viral suppression between SPNS participants and non-participants at the Meharry site. The SPNS study enrolled women of color newly who were recently diagnosed with HIV or lost to care. There was no statistically significant difference in viral suppression after controlling for demographic and clinical factors.

The New York City Department of Health and Mental Hygiene designed an HIV self-management protocol for the Ryan White Part A care coordination program. Through the protocol, staff and patients systematically identify and address patient strengths and challenges, focusing activities on building patients' capacity to manage their care.

The New York City Health Department has scaled up The Undetectables Viral Load Suppression Program, in collaboration with the program developer (Housing Works) and other stakeholders, by integrating the program into existing HIV medical case management service delivery.

Prism Health North Texas will share the challenges and successes of adapting Ryan White case management tools and strategies to a pre-exposure prophylaxis (PrEP) program for behavioral intervention. This session will describe the development and implementation of a web-based electronic and administrative tracking system developed for use by clinic case managers. This electronic tracking tool allows case managers to easily view patient data with the overall goal of improving retention in care and adherence to medication.

Caracole, an AIDS Service Organization, uses three interconnected approaches to improve retention in HIV care: housing first, harm reduction, and motivational interviewing. Clients in permanent supportive housing had high rates of viral suppression, exceeding Caracole's goal of 75%.
