Through the Southern Initiative, CAI supported four Part A agencies in the South in integrating a Community Health Worker (CHW) model to address disparities in HIV outcomes. The session will describe the development of systems to collect, report, and use real-time data to track implementation progress and client outcomes. Results reveal promising client outcomes.

This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. The New York City Department of Health and Mental Hygiene designed an HIV self-management protocol for the Ryan White Part A care coordination program. Through the protocol, staff and patients systematically identify and address patient strengths and challenges, focusing activities on building patients' capacity to manage their care.

Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations

Caracole, an AIDS Service Organization, uses three interconnected approaches to improve retention in HIV care: housing first, harm reduction, and motivational interviewing. Clients in permanent supportive housing had high rates of viral suppression, exceeding Caracole's goal of 75%.
