HIV care measures for people with HIV enrolled in the Massachusetts AIDS Drug Assistance Program (ADAP) were compared to people with HIV not currently enrolled; results indicate that enrollees have higher frequencies of engagement and retention in care and viral suppression. Evaluation of the data provides insight that enhances programming and service delivery to reduce health disparities.

The STAR Program, a Ryan White Part C program, initiated a 30-minute, weekly televised TV health magazine that engages the community across multiple social media and television platforms with a focus on health particularly HIV treatment and Prevention, reaching young adults. The HIV Prevention Certified Provider (PCP) program is an online, self-paced CME curriculum composed of five free e-learning courses in HIV prevention. Clinicians who complete the curriculum receive the HIV PCP national designation and certificate, recognizing competency in the clinical and cultural aspects of HIV prevention services, including pre-exposure prophylaxis (PrEP).

Access to telemedicine visits in a multidisciplinary urban HIV clinic allows for improved access to care for patients who have fallen out of care or those who have difficulty attending their appointments due to stigma, multiple comorbidities, or barriers related to work, transportation, or childcare.

Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

Oklahoma has been classified by the Department of Health and Human Services as one of the seven states with a high rural HIV burden. Test-and-treat protocols are feasible within high-volume HIV clinics which serve rural and underserved communities to minimize the time to the first appointment to decrease time to viral load suppression.

Undetectable=Untransmittable: individuals who maintain viral suppression don't transmit HIV to their sexual partners. Yet some providers are uncomfortable discussing the implications of viral suppression in clinical encounters. This workshop will explore approaches to reduce provider discomfort in discussing U=U and present strategies to convey this message.

This presentation assesses social determinants that affect the HIV continuum of care. Establish patterns at the economic, demographic, and sociocultural factors that may fragment or, in a worst-case scenario, interrupt access to HIV testing, biomedical prevention, follow-up to a positive diagnosis, and adherence to treatment.
