Five sites participating in the SPNS Black Men who Have Sex with Men (BMSM) Initiative are implementing the Youth-focused Case Management model of care to support behavioral health services integration to improve HIV health outcomes. This session will discuss the intervention adaptations to fit local context and population needs and early implementation experiences.

Telepsychiatry has proven to provide better access and higher-quality care to patients who need psychiatric care as well as for those who have varying circumstances that make it difficult to engage in this service. Vivent Health has successfully integrated telepsychiatry within its medical home model. With two different service delivery methods, this presentation will look at the benefits of telepsychiatry for people with HIV, as well as the unique delivery methods Vivent Health provides in Wisconsin and Colorado.

The presentation will focus on the specifics of delivering brief mental health in-clinic interventions and the development and implementation of youth peer support groups within the pediatric HIV program in the ‘hot spot' of the national HIV epidemic, Washington, D.C.

Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

This session showcases the activities undertaken by the Ryan White Wellness Center in Charleston, South Carolina, that helped eliminate viral suppression disparities among its young men of color who have sex with men.

Successes and lessons learned will be shared from three metropolitan areas on incorporating STI testing and treatment for prevention clients within an HIV medical home setting, along with how offering STI treatment impacts early identification service outcomes (e.g., HIV positivity rates, linkage to care referrals, and the rapid start of HIV treatment).

Boston Healthcare for the Homeless Program uses an innovative care model, designed from the margins, to meet the complex needs of people living at the intersections of HIV, homelessness, substance use disorder, and incarceration. It will explore culturally responsive leadership, transformational leadership, and cultural humility. Facilitators will demonstrate formal and informal leadership skills. This session will give information on the RWHAP SPNS Social Media Initiative (SMI), which used social media and digital technology to improve HIV outcomes for young people. It will provide cross-site outcomes from 10 demonstration sites. Three sites will discuss demonstration models. Resources for replicating the intervention models will be presented.

Substance abuse, interpersonal violence, depression, and HIV are part of overlapping and intersecting epidemics which adversely affect the prognosis and intensify the burden of each. This session will review existing literature on this syndemic, lessons learned in initial implementation of a screening and linkage program, and implications for practice.
