Positive Love: Improving Health Outcomes of Young, Black MSM with HIV in the Deep South
The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

National Implementation of a Text-Messaging Intervention to Improve Antiretroviral Adherence among Youth with HIV
This workshop will explore two sites' (Brooklyn and Detroit) experience with the implementation of an evidence-informed intervention for black men who have sex with men (MSM) with HIV. Proactive Clinic-Based Substance Abuse Screening and Treatment: Helping to Advance in New Directions (Helping HAND)
A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

Utilizing an HIV Clinical Pharmacist to Improve Patient Outcomes
This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. Test-and-Treat Protocol in a Rural State To Decrease Time to Undetectable Viral Load
Oklahoma has been classified by the Department of Health and Human Services as one of the seven states with a high rural HIV burden. Test-and-treat protocols are feasible within high-volume HIV clinics which serve rural and underserved communities to minimize the time to the first appointment to decrease time to viral load suppression.

Mobilizing People and Resources to Address HIV and Aging

Undetectable=Untransmittable: Walking the Walk, Talking the Talk
Undetectable=Untransmittable: individuals who maintain viral suppression don't transmit HIV to their sexual partners. Yet some providers are uncomfortable discussing the implications of viral suppression in clinical encounters. This workshop will explore approaches to reduce provider discomfort in discussing U=U and present strategies to convey this message.

Community Health Workers Improve Viral Load Suppression in Non-Treatment Adherent Populations to End the Epidemic
Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations
