In 2015, only 72% of pregnant women with HIV followed at the Grady Ponce de Leon Center returned for postpartum care within a three-month period of time. This workshop will describe actions taken to increase the number of women who kept their postpartum (fourth trimester) and ID appointments.

Despite systemwide efforts to increase capacity for quality improvement (QI) there remains challenges in bridging the gap between quality improvement concepts, best clinical practices, and client barriers to quality care. University of Mississippi Medical Center utilized known barriers to care for postpartum women with HIV and leveraged that information to schedule individualized interactions with pregnant and postpartum women with HIV. This low-cost, low-effort initiative resulted in statistically significant improvements in both retention in care and viral suppression rates in postpartum women with HIV.

Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

The Linkage to Care (LTC) Program at Denver Health/Denver Public Health is an innovative model using continuous quality improvement and community partners to close gaps in the HIV care continuum. This linkage model serves those seeking HIV prevention service as well as people with HIV seeking linkage and retention in care.

Establishing an opt-out HIV screening program in the emergency department has almost quadrupled HIV screening rates and has identified new and out-of-care cases. Utilizing a continuous quality improvement approach has allowed it to become a catalyst to improve workflows for other gaps in University Medical Center of Southern Nevada's HIV care continuum process.

The Alabama Quality Management Group (AQMG) is a consortium of quality leaders representing all Alabama RWHAP Part C and D clinics, over 80% of Alabama's HIV-diagnosed population. Data for Care Alabama (D4C-AL), a CDC-funded retention-in-care project, uses risk-stratification based on missed visits. It is currently being implemented at one site but will continue to be implemented over time.

Workshop attendees will be introduced to the end+disparities ECHO Collaborative, the largest virtual community of practice of its kind.
