Timely antiretroviral treatment (ART) initiation, linkage, and retention in care are critical keys for the optimal outcomes in patients with HIV infection. Implementing an open access process for medical, mental, and intensive case management care gives patients the ability to better achieve medication adherence and viral load suppression.

North Carolina's HIV/STD/Viral Hepatitis Unit, along with Western North Carolina Community Health Services and Carolina Family Health Center -- two federally qualified health centers (FQHCs) -- has initiated a pilot rapid antiretroviral treatment (ART) program utilizing carryover funds and a streamlined HIV Medication Assistance Program application process. Preliminary data will be shared on the success of the pilot program.

This presentation will focus on viral suppression and the New York Eligible Metropolitan Area’s efforts to address care continuum outcome disparities affecting clients of transgender experience.

This session will describe the use of motivational interviewing in coaching to improve HIV testing, prevention, and care in rural clinics and other settings. Attendees will practice using motivational interviewing for organizational change.

This session will outline establishment of community partnerships and implementation of housing, nutrition, and substance use treatment programs. It will detail contract development between the presenters’ clinic and community partners and outline how, following policy clarification notice (PCN) 16-02 guidance on allowable uses of program income, community partners are reimbursed in support of these programs.
