This includes both a recipient serving homeless clients and a RWHAP AIDS Education and Training Center that trains clinicians on strategies to serve clients experiencing homelessness. In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Boston Healthcare for the Homeless Program uses an innovative care model, designed from the margins, to meet the complex needs of people living at the intersections of HIV, homelessness, substance use disorder, and incarceration. Such models hold promise for closing HIV care and prevention equity gaps for this hyper-vulnerable group.

The panel will discuss experiences working with Ryan White and housing data, including the development of expanded housing definitions and findings from research. The workshop will engage participants to discuss what housing information is currently collected in their system and review what additional information it would be helpful to capture.

The New Jersey Behavioral Health and HIV Integration Project (NJ B-HIP) used an HIV and Behavioral Health Continuum and applied a kit of essential tools to achieve behavioral health and primary HIV care integration and improved outcomes. Specific frameworks, tools, and site-based examples will be shared, and cross-cutting issues will be discussed.

Cooper EIP Expanded Care Clinic in Camden, NJ has incorporated comprehensive Addictions Medicine (AM) care as part of its treatment model for people with HIV. A cohort of Substance Use Disorder (SUD) diagnosed patients displayed significant levels of VLS after enrolling in AM services.

This session will introduce the components of integration and highlight available resources for integrative care.

This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. Pharmacists' involvement in pre-exposure prophylaxis will also be reviewed.

This session will examine the components of a successful HIV treatment and care model targeting foreign-born and other vulnerable patient populations. Key elements of the model will be reviewed, including the role of the multidisciplinary, multicultural team, and program infrastructure supporting a regular review of patient data and clinical quality measures.

Through creative innovation, multiple approaches have been employed to assist patients with medication adherence. This presentation will look at the introduction of a clinical pharmacist in an urban, Ryan White-funded clinic, the impact on medication adherence, and the viral suppression and demographic trends observed over four years.

Boston Healthcare for the Homeless Program will share its innovative approach toward the micro-elimination of Hepatitis C (HCV) for homeless-experienced people with HIV. Team members will share data around cure rates, reinfection, and engaging with marginalized patients, and discuss leveraging existing Ryan White infrastructure and resources for such efforts.

The New York City Department of Health and Mental Hygiene designed an HIV self-management protocol for the Ryan White Part A care coordination program. Through the protocol, staff and patients systematically identify and address patient strengths and challenges, focusing activities on building patients' capacity to manage their care.
